This service is provided by our new clinical system “EMIS WEB online services”
To register for this we recommend you use Patient Access or myGP:
BOOK APPOINTMENTS, REQUEST REPEAT
PRESCRIPTIONS and VIEW YOUR TEST RESULTS
In order to have this access online, you will need to complete the attached form Registration Form
(also available at Reception) you will be asked to provide photo ID and proof of address for each adult registering.
Under 18's please see attached Proxy forms for specific age ranges:
0 to 10yrs
11 - 15yrs
Over 16's please contact Reception for a Form
If you were previously registered for this service before our clinical system change mid February 2020, you should have been contacted now with a new PIN so you can register on one of the above websites.
